Change of Ownership Form
This form is required by the Housing Authority for proof of ownership.  This will allow the Housing Authority to change rental payment to the new owner.  When the form is completed please attach proof of ownership, proof of insurance, IRS Form W-9 and mail to the Housing Authority at 1632 Yale Street, North Las Vegas, Nevada 89030.
